Vitality Circle Terms & Cancellation Policy

These Terms & Cancellation Policy govern the Vitality Circle, the $99.99/month wellness membership offered by Vita Nova Ketamine & Wellness Clinic at 513 Bayview Blvd, Canton, Baltimore MD 21224. By enrolling in the Vitality Circle, you agree to the terms below, including the three-month minimum commitment, monthly recurring billing, the monthly perk structure, and the cancellation and refund terms described in this policy. Please read this policy in full before enrolling.

1Membership Overview
What the Vitality Circle Is
The Vitality Circle is a paid monthly wellness membership offered by Vita Nova Ketamine & Wellness Clinic ("Vita Nova," "we," "us," or "the clinic"). It provides members with one included wellness perk each billing month plus always-on discounts on premium IV infusions, additional wellness services, and ketamine infusion sessions. Membership is optional. All Vita Nova services remain available to non-members at standard pricing.
The Vitality Circle is a membership and discount program only. It is not insurance, not a health plan, and not a prepaid medical services contract. Enrolling does not obligate you to receive any particular treatment, and membership does not guarantee clinical eligibility for any service.
2Membership Fee & Billing
Fee, Billing Cycle & Auto-Renewal
Monthly fee: The Vitality Circle costs $99.99 per month.
Recurring billing: The fee is charged automatically each month to the payment method you provide at enrollment. Your billing date is set by your enrollment date and recurs on the same day each month.
Auto-renewal: Membership renews automatically each month until you cancel in accordance with this policy. There is no separate renewal notice.
Payment method: You are responsible for keeping a valid payment method on file. If a payment fails, we may retry the charge and may suspend member benefits until the balance is resolved.
HSA / FSA: HSA and FSA cards may be accepted depending on your plan administrator's rules. Eligibility is determined by your plan, not by Vita Nova.

What Membership Includes
Vitality Circle membership includes two independent benefit layers that can be used on the same visit:
Part 1 — One included perk each billing month (choose one):
One standard IV infusion (any standard formula), or
Two IM shots (any two from the menu), or
Two IV pushes (any two from the menu), or
One IM shot plus one IV push (one of each).

Ketamine member pricing note: Member ketamine session pricing applies only to eligible patients following a comprehensive clinical evaluation. Ketamine infusion therapy is off-label and anesthesiologist-led. Membership does not by itself qualify you for ketamine therapy.
4Monthly Perk Terms
How the Monthly Perk Works
One perk per billing month. You select one perk option per billing month at the time of your visit. You do not need to decide in advance.
No rollover. Unused perks expire at the end of each billing month and do not carry over, accumulate, or convert to account credit or cash value.
Standard IV perk excludes premium IVs. The included standard IV perk applies to standard IV formulas only. NAD+ IV, High-Dose Vitamin C IV, and Iron IV are premium infusions and are not part of the included monthly perk — though they always receive the 25% member discount.
Personal use only. Perks and member pricing apply to the enrolled member only and may not be shared, gifted, or transferred to another person.
5Minimum Commitment
Three-Month Minimum Commitment
Vitality Circle membership carries a three-month minimum commitment beginning on your enrollment date. During this initial three-month period, the monthly fee continues to be billed and the membership cannot be cancelled for convenience. This commitment allows the clinic to reserve member benefits and priority scheduling on your behalf.
After the minimum: Once you have completed the three-month minimum commitment, your membership continues month-to-month and you may cancel at any time under the terms in Section 6.
6Cancellation
How to Cancel
When you can cancel: You may cancel at any time after completing the three-month minimum commitment.
How to cancel: Submit your cancellation request by email to info@vitanovawellnessclinic.com, by phone at (443) 563-1059, or in person at the clinic. We recommend email so you have a written record of the date.
When cancellation takes effect: Cancellation takes effect at the end of your current billing cycle. Your membership and benefits remain active through the end of the cycle you have already paid for, and you will not be billed again after that.
No partial-month billing back-outs: Because cancellation is effective at the end of the paid cycle, the fee for the current cycle is not reversed when you cancel.
7Refunds
Refund Policy
Membership fees are non-refundable. Monthly Vitality Circle fees, including the current cycle at the time of cancellation, are not refundable.
Paid-for services are honored for 30 days. If you have already paid for a specific service at the member rate that has not yet been provided at the time you cancel, that service will be honored for 30 days following your cancellation date so you receive what you paid for.
Perks have no cash value. Unused monthly perks are not refundable and carry no cash value.
Vita Nova may update Vitality Circle pricing, perk options, discount percentages, eligible services, or these terms from time to time. Where a change materially affects current members, we will provide reasonable advance notice by email or at the clinic before the change takes effect. Continued membership after a change takes effect constitutes acceptance of the updated terms. Membership pricing and benefits described on this page and on the Vitality Circle Membership page are current as of the effective date above and are subject to change.
9Eligibility & Medical Services
Eligibility & Medical Services
Wellness services such as IV vitamin therapy and IM shots are general wellness services and are not a substitute for medical care or treatment of any medical condition.
Ketamine therapy is an anesthesiologist-led, off-label medical service. Member session pricing applies only after a comprehensive clinical evaluation determines you are an appropriate candidate. Membership does not guarantee eligibility, and not all patients who complete evaluation are offered treatment.
Age: Membership is available to adults 18 and older.
Clinical decisions always take precedence over membership benefits. A provider may decline or modify any service on clinical grounds regardless of membership status.
10General Terms
General Terms
Non-transferable: Membership, perks, and member pricing are personal to the enrolled member and cannot be transferred or assigned.
Governing law: This policy is governed by the laws of the State of Maryland.
Entire agreement: This policy, together with the Vitality Circle Membership page and any enrollment terms you accept at sign-up, constitutes the agreement between you and Vita Nova regarding the membership.

After your cancellation takes effect at the end of that cycle, member pricing and perks no longer apply, and services return to standard non-member pricing. Any service you already paid for at the member rate but have not yet received is honored for 30 days after cancellation.
