Adults 18 and older with major depressive disorder, bipolar depression, or postpartum depression who have not responded adequately to two or more treatment trials at adequate dose and duration; PTSD, OCD, or anxiety disorders with comorbid depression that have not responded to standard care; and chronic pain conditions — neuropathic, fibromyalgia, CRPS, migraine, sciatica, pelvic, or post-surgical pain — refractory to conventional management. A current medication list, prior treatment history, and relevant diagnoses are the most useful items to send.

Uncontrolled hypertension or unstable cardiovascular disease; a history of unmanaged psychosis or active mania; elevated intracranial or intraocular pressure; active, untreated substance use disorder; pregnancy.
