Ketamine for
OCD in Baltimore

When SSRIs and ERP haven't broken the loop — a different circuit, a different mechanism

Ketamine for OCD Baltimore costs $499.99 per session ($375 for Vitality Circle members) and is an off-label IV infusion evaluated for obsessive-compulsive disorder that hasn't responded adequately to SRIs and ERP therapy. OCD is driven by dysfunction of the cortico-striato-thalamo-cortical (CSTC) circuit, a glutamate-driven feedback loop — ketamine's NMDA blockade targets that glutamatergic hyperactivity directly, where SSRIs reach it only indirectly. The evidence is promising but small and mixed: in the 2013 Rodriguez randomized crossover trial, obsessions eased within hours of a single infusion, but durability remains the open question, so ketamine is positioned to complement ERP — never replace it. Every infusion is delivered as anesthesiologist-led care, under the medical direction of Dr. Brijen Joshi MD, a Johns Hopkins anesthesiologist, with a Certified Registered Nurse Anesthetist (CRNA) on-site. Self-pay with an itemized superbill you can file with your insurance; not everyone is a candidate, and you'll be told clearly.

OCD & Ketamine at a Glance
1
Unique circuit — not depression: OCD involves the CSTC loop — a glutamate-driven circuit distinct from the limbic and monoamine pathways of depression. This is why ketamine, an NMDA glutamate blocker, is mechanistically relevant where SSRIs reach the circuit only indirectly
2
40–60% of OCD patients do not achieve adequate relief from first-line SSRI treatment and ERP therapy — the two gold-standard approaches that must be tried before ketamine is evaluated
3
Landmark evidence: Rodriguez et al. 2013 — the first double-blind, placebo-controlled RCT of ketamine for OCD — found significant symptom reduction within hours of a single infusion — though effects faded within days to two weeks for most, so durability remains the open question
4
Off-label for OCD: ketamine is not FDA-approved for OCD — off-label prescribing is a legal, standard medical practice, here delivered as anesthesiologist-led care, under the medical direction of Dr. Brijen Joshi MD, a Johns Hopkins faculty anesthesiologist
5

01
Understanding the Condition

What Is OCD — And Why It's a Circuit Problem

Obsessive-compulsive disorder is characterized by recurrent, intrusive obsessions — unwanted thoughts, images, or urges that cause significant distress — and repetitive compulsions performed to reduce that distress. The compulsion provides short-term relief but reinforces the cycle, making the next obsession more likely and more distressing.

OCD is not a monoamine disorder in the way depression is. It is fundamentally a circuit disorder — specifically, a dysfunction of the cortico-striato-thalamo-cortical (CSTC) loop. This is why SSRIs provide only partial relief for many OCD patients: they are acting on the wrong primary neurotransmitter system. The dominant neurotransmitter in the CSTC circuit is glutamate — which is where ketamine's mechanism becomes directly relevant.

The CSTC Circuit — How OCD Gets Stuck
Orbitofrontal Cortex

Generates obsessive thought · Signals "something is wrong"

→
Striatum (Caudate)

Fails to filter excessive OFC signals · Passes urgency forward

↑↓
Cortex (Loop)

Receives amplified signal · Generates compulsion to neutralize

←
Thalamus

Amplifies signal · Sends it back to cortex · Loop continues

In OCD, this loop becomes pathologically hyperactive — driven by excess glutamatergic signaling at each node. The result is the "stuck" quality that OCD patients describe: a thought that cannot be dismissed, an urge that cannot be resisted. Ketamine blocks NMDA glutamate receptors, directly dampening the hyperactivity of this circuit. SSRIs act on serotonin, which modulates the circuit indirectly — explaining why they provide only partial relief in many OCD patients.

OCD Subtypes Evaluated at Vita Nova

OCD presents across several distinct subtypes — all four major presentations are evaluated for ketamine at Vita Nova when first-line treatments have not produced adequate relief.

🧼
Contamination OCD
Obsessions: Fear of germs, disease, contamination from objects, people, or environments
Compulsions: Excessive hand-washing, cleaning rituals, avoidance of "contaminated" situations
🔒
Checking OCD
Obsessions: Fear of harm through negligence — unlocked doors, gas left on, accidents
Compulsions: Repeated checking, seeking reassurance, retracing routes or actions
🔢
Symmetry & Ordering OCD
Obsessions: Sense of "not just right" — objects or arrangements feel wrong until symmetrical
Compulsions: Ordering, arranging, repeating actions specific numbers of times until "right"
💭
Intrusive Thought OCD
Obsessions: Unwanted intrusive thoughts of harm, violence, or moral transgression — distressing precisely because they contradict the person's values
Compulsions: Mental rituals, prayer, avoidance, seeking reassurance, thought suppression
02
Gold-Standard First-Line Treatments

ERP & SSRIs First — Ketamine When They Haven't Been Enough

Ketamine for OCD is evaluated at Vita Nova only for patients who have already attempted first-line treatments. ERP and SSRIs are the gold standard — with the strongest evidence bases. If you have not yet tried these, they should be your first step.

✅
First-Line OCD Treatments — Try These Before Ketamine
The following are the gold-standard evidence-based treatments for OCD. Both are required prior to ketamine evaluation at Vita Nova. Documentation is reviewed during the clinical evaluation.
ERP — Exposure & Response Prevention

The gold-standard psychotherapy for OCD. Involves graduated exposure to feared situations while preventing the compulsive response. An adequate course means approximately 12–20 structured sessions with a trained ERP therapist.

SSRI Pharmacotherapy

FDA-approved SSRIs for OCD include fluoxetine, fluvoxamine, paroxetine, sertraline, and the TCA clomipramine. OCD typically requires higher SSRI doses than depression and a trial of at least 8–12 weeks at therapeutic dose.

Ketamine is not a first-line OCD treatment. It is evaluated for patients who have not achieved adequate response from the above approaches. For OCD therapist referrals in Maryland, the IOCDF (iocdf.org) maintains a therapist directory.
03
Why Ketamine May Work for OCD

The Mechanism — Glutamate, NMDA & the CSTC Loop

OCD is the condition for which the glutamate hypothesis is the primary neurobiological model. The dominance of glutamate in CSTC circuit dysfunction makes NMDA blockade mechanistically more directly relevant to OCD than to almost any other mental-health condition ketamine is evaluated for.

💊
SSRIs — Indirect CSTC Modulation
  • Modulate serotonin reuptake — serotonin has indirect modulatory effects on the CSTC circuit but is not the primary driver
  • Require 8–12 weeks at therapeutic dose before OCD response can be assessed — often at doses higher than used for depression
  • 40–60% of OCD patients achieve inadequate relief even after adequate SSRI trial
  • Do not directly address glutamatergic hyperactivity in orbitofrontal-striatal-thalamic circuits
⚡
Ketamine — Direct CSTC Glutamate Blockade
  • Blocks NMDA glutamate receptors in the orbitofrontal cortex, striatum, and thalamic circuits — the primary nodes of CSTC hyperactivity in OCD
  • Rapidly reduces glutamatergic hyperactivity in the CSTC loop — potentially interrupting the pathological feedback that generates obsessions
  • In the Rodriguez et al. 2013 RCT, significant OCD symptom reduction (Y-BOCS score) was observed within hours of a single infusion
  • Mechanism is distinct from SSRIs — explaining why it may be effective in patients for whom SSRI treatment has failed

Clinical Evidence

The Research Behind Ketamine for OCD

The evidence base for ketamine in OCD is anchored in one small double-blind crossover trial — rigorous by design but far smaller than the treatment-resistant depression evidence base, with mixed follow-up results and durability unresolved. The glutamate hypothesis is well-established. Larger trials are ongoing.

Landmark RCT · 2013
Rodriguez et al. — Randomized Crossover Trial (n=15)
The first randomized, double-blind, placebo-controlled crossover study in drug-free adults with near-constant obsessions. Obsessions eased during and within hours of a single IV ketamine infusion versus saline; at one week, 50% of ketamine-first patients met response criteria versus none on placebo. For most, effects faded within days to two weeks — durability is the open question this trial left unanswered.
PMID 23783065 · Neuropsychopharmacology, Vol 38(12)
Mechanism Study · 2011
Pittenger et al. — Glutamate Hypothesis of OCD
Comprehensive review establishing the glutamate hypothesis of OCD — documenting elevated glutamate in the CSTC circuit, particularly in the orbitofrontal cortex and caudate nucleus. Provided the neurobiological rationale preceding the clinical ketamine trial design.
Peer-reviewed review, 2011
Durability · Open Question
Rodriguez et al. 2016 — Ketamine + Exposure Therapy
Because single-infusion effects fade, the same Columbia group tested whether pairing IV ketamine with a brief course of exposure-based CBT could extend the benefit. In this small open-label trial, most participants held meaningful improvement through two-week follow-up. It is preliminary and uncontrolled — but it is the mechanistic reason Vita Nova positions ketamine as an adjunct to ERP, never a substitute.
PMID 27046314 · Open-label trial, 2016
Evidence Context
Maturity vs. TRD — What This Means Clinically
The OCD evidence base is earlier-stage than treatment-resistant depression — one pivotal RCT rather than multiple replicated trials. This is reflected in how Vita Nova positions ketamine for OCD — as a carefully evaluated option for patients with documented treatment-refractory OCD. Individual results vary more than in TRD populations.

Ketamine has demonstrated rapid OCD symptom reduction in peer-reviewed clinical research. The landmark study is Rodriguez et al. 2013 (PMID 23783065) — a randomized, double-blind, placebo-controlled crossover trial in drug-free adults with treatment-refractory OCD. Obsessions eased within hours of a single IV ketamine infusion, and half of ketamine-first patients still met response criteria at one week — but effects faded for most within days to two weeks, so durability is the open question. Ketamine for OCD is off-label, and individual results vary significantly. Eligibility requires a comprehensive clinical evaluation at Vita Nova, including documentation of prior SSRI and ERP treatment attempts.
No. Ketamine infusions for OCD are off-label — not FDA-approved for this indication. The only FDA-approved pharmacological OCD treatments are SSRIs (fluoxetine, fluvoxamine, paroxetine, sertraline) and the TCA clomipramine; ERP is the gold-standard first-line psychological treatment. Off-label prescribing is a legal and standard medical practice when a licensed physician determines it clinically appropriate based on the published evidence. At Vita Nova, ketamine for OCD is evaluated only for patients who have not achieved adequate response from these established first-line approaches, and all care is anesthesiologist-led care, under the medical direction of Dr. Brijen Joshi MD.
OCD involves hyperactivity of the cortico-striato-thalamo-cortical (CSTC) circuit — a glutamate-driven feedback loop between the orbitofrontal cortex, caudate nucleus, thalamus, and back to the cortex. This loop becomes pathologically active in OCD, generating intrusive obsessions and the compulsion to neutralize them. Ketamine blocks NMDA glutamate receptors in these circuit nodes, reducing the glutamatergic hyperactivity that maintains the loop. This is mechanistically distinct from SSRIs, which modulate serotonin and engage the CSTC circuit only indirectly — which is part of why ketamine may help when SSRIs have not.
Yes. At Vita Nova, ketamine for OCD is evaluated for patients who have not achieved adequate response from an adequate SSRI trial at therapeutic OCD doses for at least 8–12 weeks, and an adequate course of ERP therapy with an OCD-specialist therapist (approximately 12–20 structured sessions). ERP and SSRIs are the gold-standard first-line treatments for OCD. Ketamine is not appropriate for patients who have not yet attempted these approaches. Documentation of prior treatment history is reviewed during the clinical evaluation.
Yes. ERP remains the most effective long-term psychological intervention for OCD and should be continued alongside any ketamine pathway. Ketamine may reduce OCD symptom burden — particularly obsession severity — which could make ERP exercises more accessible by lowering the baseline anxiety that makes exposure tasks difficult. However, ERP is not replaced by ketamine. If you do not currently have an OCD-specialist ERP therapist, the IOCDF therapist directory at iocdf.org can help identify qualified therapists in Maryland.
Most patients begin with an induction series of 4 to 6 IV ketamine infusions over roughly two to three weeks. This series is designed to establish whether you respond and to consolidate that response. After induction, maintenance infusions are scheduled individually based on how durable the effect is — some patients need them every few weeks, others less often. There is no single number that applies to everyone; your provider sets and adjusts the plan based on your response and your continued ERP work. Ketamine is delivered alongside ERP and your existing treatment, not as a replacement for them.
Sometimes, indirectly. Iron-deficiency anemia causes fatigue, brain fog, restlessness, and poor concentration — symptoms that can amplify the exhaustion of living with chronic OCD and make compulsions harder to resist. Iron is also a cofactor in the synthesis of dopamine and serotonin. An Iron IV does not treat OCD and is not part of the ketamine protocol — but if bloodwork confirms iron-deficiency anemia, correcting it can remove a physical drain that sits on top of the disorder. Ask your Vita Nova provider about checking ferritin and a CBC; when deficiency is confirmed, an Iron IV infusion restores stores faster than oral iron.
In the Rodriguez et al. 2013 study, ketamine produced significant reductions in overall OCD symptom severity (Y-BOCS). The obsession dimension — intrusive thoughts, images, and urges — showed particularly pronounced early response, consistent with ketamine's direct effect on the glutamatergic circuits generating the obsessions. The compulsion dimension showed improvement but with a less pronounced early effect — reflecting that compulsive behaviors, once learned, require continued ERP work for full resolution. All four major OCD subtypes are evaluated at Vita Nova. Individual responses vary significantly.
Most patients do. Ketamine is used off-label for OCD, usually alongside existing treatment such as an SSRI and exposure and response prevention therapy, not instead of it. Bring a full medication list to your evaluation, and don't stop or change any medication without talking to your prescriber.
