When you can't feel what you know you should — and existing treatments have flattened you further
Ketamine for emotional numbness Baltimore costs $499.99 per session ($375 for Vitality Circle members) and is an off-label IV infusion evaluated for anhedonia, restricted affect, and SSRI-related emotional blunting. Emotional numbness is a symptom, not a diagnosis: it is usually a feature of treatment-resistant depression, PTSD, or the antidepressant itself, so it is evaluated inside a full depression workup rather than as a standalone indication. Ketamine acts through two pathways SSRIs do not engage — NMDA blockade promoting synaptogenesis in reward circuits, and opioid receptor modulation influencing hedonic tone — and in NIMH research reduced anhedonia rapidly after a single infusion (Lally 2014). Every infusion is delivered as anesthesiologist-led care, under the medical direction of Dr. Brijen Joshi MD, a Johns Hopkins anesthesiologist, with a Certified Registered Nurse Anesthetist (CRNA) on-site. Self-pay with an itemized superbill you can file with your insurance; not everyone is a candidate.

"My depression is better on the medication — I know it is. But I can't cry at my own father's funeral. I can't feel excited about things I used to love. I look at my kids and I feel nothing. I'm not sad. I'm just... absent."
This experience has a name — and it has a neurobiological explanation. Emotional numbness, disconnection, and the inability to feel pleasure or emotional range affect a large proportion of people living with depression, PTSD, and those on long-term antidepressant therapy. It is not ingratitude. It is not a character flaw. It is a measurable, documented phenomenon with identifiable causes — and for some patients, a care pathway that reaches it when existing treatments have not.
Emotional numbness is not a single diagnosis — it is a symptom cluster that appears across multiple conditions and, critically, as a direct side effect of some of the most commonly prescribed mental health medications. Understanding the source of the numbness is the first step to finding what might address it. At Vita Nova, the clinical evaluation is designed to identify which of the following sources — or combination of sources — is most relevant to your specific experience.
Anhedonia — the inability to experience pleasure from activities, relationships, food, or achievements — is a core feature of major depression and treatment-resistant depression. It reflects hypoactivity of the brain's reward circuits: the nucleus accumbens, ventral tegmental area, and prefrontal cortex. Standard antidepressants that target serotonin do not directly engage these reward circuits — which is why anhedonia often persists even when other depression symptoms improve on SSRIs.
TRD / Major DepressionEmotional numbing is a core DSM-5 symptom cluster of PTSD (Criterion D) — characterized by restricted range of affect, inability to experience positive emotions, feelings of detachment from others, and a sense of a foreshortened future. The emotional numbing in PTSD is a protective neurobiological response to overwhelming trauma — the brain dampens emotional reactivity to prevent re-experiencing. It is often the most persistent PTSD symptom, outlasting hyperarousal and re-experiencing in many patients.
PTSDSSRI-induced emotional blunting is a pharmacological side effect — caused not by the condition but by the treatment. By broadly increasing serotonin availability across the brain, SSRIs dampen not only negative emotional reactivity but also positive emotional range. Patients describe being unable to cry at funerals, unable to feel joy at milestones, and unable to connect emotionally with loved ones — while their depression scores improve. This is documented and measurable: in a 2017 survey of 669 patients on antidepressants, about 46% reported emotional blunting.
SSRI Side Effect — IatrogenicDepersonalization (feeling detached from one's own thoughts and feelings) and derealization (feeling detached from one's surroundings) produce a persistent sense of emotional absence — as if watching one's own life from behind glass. These experiences appear in depersonalization-derealization disorder, as features of severe anxiety and depression, as trauma responses, and as transient effects of certain medications. The emotional disconnection is real — not performance, not laziness, not ingratitude.
Depersonalization / DerealizationSSRI-induced emotional blunting is the single most common form of emotional numbness seen in patients who arrive at Vita Nova asking about ketamine. It is also the least discussed in clinical settings — many patients are told their numbness is "residual depression" when it is in fact a documented pharmacological effect of their treatment.

Ketamine is the only treatment evaluated at Vita Nova that operates through two distinct mechanisms both relevant to emotional numbness. For SSRI-induced blunting specifically, the secondary opioid pathway is the most clinically interesting — because it reaches hedonic circuits that serotonergic medications do not.

The evidence base for ketamine's effect on anhedonia — the core measurable component of emotional numbness — is embedded within the broader TRD literature, with one pivotal study examining anhedonia as a primary outcome.

Several wellness services at Vita Nova address the physiological foundations of hedonic capacity — the neurological energy, receptor sensitivity, and neurotransmitter cofactors that support the brain's ability to feel. Vitality Circle members receive significant discounts on all of the below.
NAD+ is the coenzyme central to mitochondrial energy production — including in the reward circuits whose hypoactivity underlies anhedonia. Mitochondrial dysfunction in the nucleus accumbens and prefrontal cortex is associated with reduced hedonic capacity. IV NAD+ supports the cellular energy substrate that reward circuit function depends on, complementing ketamine's synaptogenic mechanism.
Members: 25% off all dosesMagnesium modulates NMDA receptor sensitivity — directly relevant to ketamine's primary mechanism. Magnesium deficiency increases NMDA receptor hyperexcitability, which can impair reward circuit plasticity. Restoring magnesium levels supports NMDA receptor function and the synaptic plasticity that ketamine aims to promote.
✅ Included for Vitality Circle members (2 shots/month)B12 and folate are essential cofactors in the methylation pathways that produce dopamine, serotonin, and norepinephrine — the neurotransmitters of reward, motivation, and emotional engagement. Long-term SSRI use is associated with B12 depletion. Restoring B-vitamin levels supports the neurotransmitter synthesis pathways underlying emotional range.
✅ Included for Vitality Circle members (2 shots/month)Iron is a required cofactor for tyrosine hydroxylase — the rate-limiting enzyme in dopamine synthesis, the neurotransmitter of reward and motivation. Iron-deficiency anemia lowers dopamine output and adds a fatigue, low-motivation, and poor-concentration layer that overlaps heavily with anhedonia and is easily mistaken for it. Iron does not treat emotional numbness, but when labs confirm deficiency, an Iron IV restores stores faster than oral iron. Ask about checking ferritin and a CBC.
Members: discounted pricingA targeted blend addressing neurological function and immune dysregulation — elevated inflammatory markers are associated with anhedonia and reward circuit hypoactivity in depression. The anti-inflammatory and neurological support components complement the emotional restoration that ketamine's synaptogenic mechanism aims to achieve.
✅ Included for Vitality Circle members (2 shots/month)Clinical note: These wellness services are not treatments for emotional numbness and are not part of Vita Nova's ketamine clinical protocols. They are offered as general wellness support addressing physiological factors associated with anhedonia and reward circuit hypoactivity. Any changes to existing mental health medications — including SSRIs — must be made only under the direction of your prescribing physician. Do not discontinue or taper medications without physician guidance.
Emotional numbness and disconnection are disproportionately prevalent in veterans, first responders, and healthcare workers — populations where emotional suppression is often occupationally reinforced and where PTSD-related numbing and long-term antidepressant use are common. The Community Heroes program exists to ensure cost is not the barrier.
